Risk Assessment, Confirmation Form and Insurance

Booking Procedure

All potential studio bookings will need to complete the following documents before their booking can be confirmed. (Follow the links below to complete the documents.)

2.  The following document needs to be completed at least 3 days prior to the booking to allow for your booking to go ahead.:

3. We may request insurance documents from you, such as Public Liability Cover and Rigging Insurance- if you are not a DI Member, or if you are engaging in Aerial work. Our Venue Officers will let you know what to send.
